Chesapeake-Potomac Home Health Agency (CPHHA) is dedicated to providing quality, cost-effective home health services to the residents of Charles, Calvert and St. Mary's Counties. Under the orders of a physician, staff members are assigned to begin services based on specific patient needs and geographic area.
A multi-disciplinary team treats the patient in their own homes, helping to improve the individual’s clinical and functional status. Nurses, physical therapists, occupational therapists, speech pathologists, medical social workers and home health aides are all available to assist patients, provide education and provide medical care. Patients are usually followed for approximately four to six weeks and discharged from services after making appropriate community referrals. Services at the Chesapeake-Potomac Home Health Agency are covered by Medicare, Medicaid and most insurances. Because coverage for home health services varies from policy to policy, CPHHA staff members are pleased to contact insurance companies to determine each patient’s level of coverage.
